Scheduling

The first step is booking an appointment, of course.
Check in with our staff or look online to see what our availability is and when you’re available to book.

Our regular business hours are 8:30AM – 5:30PM Monday through Thursday and 8:30AM – 1:00PM on Friday. We are also out of the office for lunch from 1:00PM to 2:00PM Monday through Thursday.

Identification

Once you’re all booked, you can start the intake process.
Follow the input sections and upload links below to submit pictures of both your ID card and your active insurance. If you have a secondary insurance, please make sure to provide that, as well.

We’re located in Sherman Oaks, on Van Nuys Blvd right off the 101. Our cross streets are Van Nuys and Huston, right across the street from the Sherman Oaks Hospital.

Parking

There is covered parking in our lot with one entrance on Van Nuys and one located on Huston. The parking fee is $1 for every 20 minutes parked, capping out at $9 for the day. We do not validate parking in our office, but there are meters located on Huston if that is preferred.
